Endoscopic Variceal Ligation(EVL):Another powerful tool for treating esophagogastric varicose veins

Ms.Huang(preudonym)has a history of liver cirrhosis for many years and has undergone twice Endoscopic Variceal Ligation(EVL)due to esophageal variceal bleeding(EVB).After discharge,Ms.Huang did not pay enough attention to care of her condition and did not promptly review her gastroscopy.

Recently,Ms.Huang often experienced dizziness,abdominal pain,dry and bitter mouth,poor appetite,and poor sleep at night.Her symptoms persisted and could not be relived,seriously affecting her quality of life.Therefore,she came to the Digestive Department of Jiangxi University of Traditional Chinese Medicine Affiliated Hospital for inpatient treatment.After admission,relevant tests and examinations were improved,including painless gastroscopy,which confirmed the presence of varicose veins in the esophagus and gastric fundus.

After undergoing painless gastroscopy and ruling out surgical contraindications based on Ms.Huang’s condition and clinical symptoms,Xie Mingjun,Deputy Director of the Digestive Department of Jiangxi University of Traditional Chinese Medicine Affiliated Hospital,fully communicated with the patient and their family members and conducted a risk assessment.The family members agreed to undergo esophageal variceal ligation(EVL).

Xie Mingjun used COOK six link ligation device for ligation treatment,with a total of ligations,and the surgical process was smooth.After the surgery,bring the patient back to the ward and instruct them to fast for 24-48 hours,then eat liquid or semi liquid food,gradually transitioning to soft food,and closely monitor the patient’s condition changes.

Esophageal variceal ligation(EVL)refers to the use of an elastic rubber ring to ligate the root of a varicose vein under the guidance of an endoscope,causing ischemia,necrosis,and detachment,resulting in varicose vein occlusion,controlling active variceal bleeding,and quickly eliminating varicose veins.Esophageal variceal ligation(EVL)has the advantages of minimal trauma,fast surgical recovery,and high safety.It has a significant effect on emergency or elective treatment of patients with esophageal varices and is an effective method for preventing esophageal variceal bleeding.

It is reported that esophagogastric varicose veins are one of the main manifestations of portal hypertension,and 95% of portal hypertension is caused by various causes of cirrhosis.The most serious complication of esophagogastric varicose veins is rupture and bleeding.When a patient experiences bleeding from esophageal and gastric varices,the bleeding may occur rapidly and in large quantities.At this time,the patient may exhibit severe hypotension  or hypovelemic shock,which may even affect their life.Therefore,if patients with liver cirrhosis exhibit black stools and vomiting blood,they should seek medical attention promptly.
